THE ONLINE APPOINTMENT SYSTEM HAS BEEN TEMPORARILY DISABLED.  WE APOLOGISE FOR ANY INCONVENIENCE THIS MAY CAUSE.

Only patients with pre-booked appointments can access the surgery. All paper prescriptions can be collected from Fawdon site. 

Access to Fawdon is restricted to patients with pre-booked appointments only, as well as prescription collection.  Access to Kingston Park is restricted to patients with pre-booked appointments only

From Monday 16th March all new appointments booked with a GP will be telephone consultations. During this period we may experience a higher volume of calls, if you are unable to reach the Practice via telephone then please use our eConsult system. 

Fitnotes are not required for the first 7 days of absence from work. If you need an isolation note for your employer these can be obtained from www.111.nhs.uk/isolation-note/

Online 'Pre-Registration' With The Practice

We are unable to accept your registration until you attend the surgery with all your completed documentation.

You can complete the forms online and send it to us but you will need to call into the surgery within 5 working days to sign your forms, provide proof of ID and discuss registration requirements.  Please allow 1 working day after completing your pre-registration online forms before attending the surgery as we need time to prepare your paper work.

 

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
